In the Department of Healthcare Professions' pursuit to ensure patient safety and quality of healthcare services provided, the department mandates all healthcare facilities in the State of Qatar (effective 1 January 2020) to have a process/policy to record and report all sentinel medical events (i.e. serious unanticipated events in a healthcare setting that result in adverse patient outcome) that are encountered during the provision of patient care at their institutions.

The process/policies developed shall include, at least, the following:

  1. Formation of an Adverse Medical Outcome Committee (AMOC) to whom all sentinel medical events shall be reported. The committee shall meet, as appropriate, to thoroughly investigate the sentinel event, identify its root-causes and issue recommendations to prevent its recurrence.

  2. Following a defined procedure for reporting the sentinel event to the AMOC, including the reporting channels and formats healthcare practitioners/patients may use.
