Subject: Missing of Narcotic Prescription (Pink Color)

After Compliments,

We have been informed that a Narcotic Prescription bearing Sr. No. 84701 is missing from a prescription book issued to one of the MOH institutions. Please do not dispense any medicine if anyone present this prescription. If this prescription has already been dispensed, please let us know the date and send us a copy of the prescription with the patient ID.

We request all of you to always ensure that the prescriptions are genuine before dispensing of any medicine.
