This contract risk sign-off form is intended for use by an in-house legal department (and other relevant approving authorities within an organisation) in relation to agreements for the supply or receipt of IT services. It has been drafted with IT outsourcing and managed services particularly in mind but can be adapted appropriately for use with other types of IT services agreements.

The form will need to be amended as appropriate depending on whether the user organisation is the recipient or supplier of the services. In most instances, where there are both supplier and customer options, the customer option is specified second and marked with an asterisk (*). The sections that are identified in underlined italics will needed to be pre-populated by the legal department so as to include the user organisation's standard position in relation to various matters, including payment terms for example.

As the form is detailed, it would typically be used for higher value and/or higher risk deals. A simplified version could be used for lower value/risk, less complex deals.
